How is the total time online feature figured? Is it done by the actual time you are on the message boards logged into the site, if you click on posts to read them and an average per thread read, or something else?

Its the time you "cookie" tells the database you are online. If you have a broadband connection you can leave it on the "home.php" page and you'll be on 24/7 since the page keeps reloading.
Essentially a meaningless statistic, and, frankly, nobody's business.

I have noticed that when your computer starts to idle it will not count the time in idle, only the time it is active and logged in while on dbstalk.com
Sure.
I have a TV that tracks hours of use. Only, it doesn't count the time when it is on but no one is watching.

So logout... Why would you think there would be any privacy? Even if they remove the number, they will still keep track of it in the database.
