Adding a receiver. Adding a switch. Adding a charge?

I'm a little lost.
I have an hd dvr & 2 hd receivers.
4 lines coming into the house. No switch.

If I want to add another hd receiver (H20), I would need a switch, right?
Do I have to pay for the switch or the installation of it?
(I am NOT a new customer.)

you would pay for the new H21 (99 + 99 install) and the switch would be included.
