I have set up a home distribution network where I have the following inputs :
(a) The SD output from my VP622 running into a Channel Vision combiner
(b) The Zone 2 output from my Pioneer VSX-94TXH A/V receiver being fed into a CE Labs 1001ST modulator - the coax output from the modulator runs into the same combiner as in (a) above
(c) The output from the combiner is fed into a Channel Vision splitter which feeds into multiple rooms

I am trying out an initial test now where all I have as input is the Pioneer's Zone 2 output being modulated and fed into the combiner and then being split into one TV.

I have tried using both UHF and Cable options on the modulator and tried different ranges of channels but all I get is snow on the one TV that my splitter feeds currently (which is a Samsung LN4066F).

The maximum run of any of the coax cables is a 150 feet.

My questions :
(a) In a setup such as the above, should I be using "Cable" or "UHF" channels in the modulator ?
(b) Related to this, should the output from the splitter go into the "Cable" or "Air" coax inputs on the Samsung TV ?

Am going nuts trying to work the different combinations so any help you can provide would be very useful - thanks in advance

Generally, you need to pick one or the other (cable or air), and set all devices to the same setting. Then, make sure that the channels you select to output your video on are at least 2-3 channels away from any adjacent channels. This is necessary because most lower-end modulators will bleed over onto the adjacent channels.

So, for example, your Dish receiver is set to Air and channel 60, so you'd set your Channel Vision mod to Air and channel 62. Set your TV to Air and do a tuner scan (or manually add the channels), and you should pick them both up.
