They make the money off of the programming over time. It is their promotion in which to get you as a customer, their cost of doing business. They make you sign up for a commitment of a year to make up for a lot of that cost. After they have you as a customer for so long then they will start making money off of you. Its been said before that it takes anywhere between 2-5 years before they break even on a customer. It just depends on the promotion given to the customer and what programming packages the customer chooses.

Generally Directv and DISH Network have a cost of aquisition for new customers of $300-500 so that free equipment and installation really isn't free. It just takes years for it to be paid off in subscription fees, that is why the $10/month customer is not worth as much as the $100/mo. customer.
