My mom received a printed channel guide in her welcome packet when she signed up with Dish a year ago, but it's now out of date. (She doesn't have Internet access.) Can she get Dish to send her a new channel guide (perhaps by calling)?

That's the first thing I tried. I see no links to channel guides on DishTV.com.

That guide is still not quite up to date. No Epix for instance.

You do realize that in is kind of environment, as soon as something is printed, its out of date.

Aha -- thanks. Meanwhile, I learned her provider is CenturyLink, and DL'd a channel guide from their site.
