You're billing period is from the 14th to the 13th of the next month.
You upgraded on 8/18, which is in the middle of the billing period.
Therefore, you were refunded for 8/18 til 9/13 for America's top 180 which you already paid for, and billing for Gold for that same period which you hadn't paid for before.
Basically you're being billed the difference in cost for that portion of the month (65.30 - 47.88 = 17.42. You'll notice that you were refunded part of the old HD package as well (8.70), so basically you're being charged an extra $8.72 for the difference in rates for the period of 8/18 til 9/13.

The 74.99 is for the next billing period (9/14-10/13).

Yep. He cancelled AT180 and upgraded to DishHD Gold.

The partial month is a refund for waht he paid in advance for AT180 plus the charge for DishHD Gold for that period plus other charges effective with the date of his upgrade.

We generally pay a month in advance. Just like for telephone service (although pay as you go is coming for E*, it still is a pay before you view service).
