Monthly fee for DVR service is $5.99 (Unless you get the top tier programming)
Lease fee for each receiver after the first is $4.99
Month fee for HD programming is $9.99
Monthly programming ranges from $29.99 to ~$99.99
As for the DVR costs, wrangle until you get it at the price you want. Starting point is $299, many get it for free or a token shipping charge. but you likely have to start with a larger monthly programming package or some sports subscriptions.

do you have to pay the $10 for the HD package if you want HD or is it just a package for a few more HD channels?

If they offer your local channels and they are available in HD, then you will receive those as well as any local sports networks and premium channels that you get within any of your total choice packages and premium packages. The HD package includes those HD channels outside of those locals, premiums and sports. THey include HDNet, Discovery HD among others...
