Based on a previous thread from adbs2007, I put together a simple survey to see if we could identify simple issues that are truly broad-based issues or are hardware/software specific. It should take you a few minutes to complete.
I tried to put the stuff that was top of mind but the answers are NOT comprehensive nor are the questions.

I have never reformatted. Just RBR once or twice a week. There is no option for that. It's not a 30 second "skip", it's a 30 (?) second "slip", which is the thing I like least.
That took about one minute!
Well worth the time if enough people provide responses and the right people at get the results to look at. Thanks.

Excellent job on the survey. I am coming up on one year of having my R15-500. I have never done a reformat, maybe 6 RBR due to the black screen, and I have never had a recording I couldn't view. My only complaint is slowness to changing channels.

Glad to see some good data gathering like this. Maybe there will be enough for a good submission to DirecTV (even though they already know the issues).
For my case, I filled out the survey, and will simply pass along the answer I left for the last question...
9. Which feature has been improved the most over the life of your R15 ownership?
None. I've had it for two weeks. The bugs and features I lost with this "upgrade" have made me make the decision to go back to my DirecTiVo.
Any possibility of making the first question a checkbox (vs. a radio button)? I have two R15s - a 300 and a 500 - and they have their own set of issues. Or should I submit two surveys?

You should submit two surveys -- one for each setup. This will allow us to see the unique problems. I appreciate you taking the time to fill it out to get some good data.

During the 16 months I have never had to reformat my R15-500 either (so I did not answer Q.7), and the last time I had to RBR was just after the last software update.
You also need to add "No problems" for each question.
You're right. I guess that this did not enter my mind for some of the questions, such as RBR. However, since I have already started I will assume that the skips are "no problem" or "have not done it".
