I'm in Chicago, I currently get 72 and 61.5 with one dish.

My brother has an older set-up - 119/110 and 61.5, but is now missing a lot of HD channels that I have (we're both on AT200 + HD)

Would the existing dish 500 pick up 72 and 61.5?
Or would it be easier and better to repoint 61.5 to 129 and leave 119/110 as is?
The goal is not to spend any money on a larger dish.

Yes a dish 500 will pick up 61.5 and 72.7 reasonable well. Which satellites you would want to point at depends on where your locals are transmitted from.

He already has a dish aimed at 61.5, why not just aim the 110/119 dish 500 at 72.7 then he should have all the programming.

Aiming at 61.5 and 72.5 would lose the SD versions of local channels in HD. But getting the channels in HD makes those SD versions less important.

Except for when rain fade affects the HD locals. It's a nice feature for the receiver to switch to the SD equivalent during storms. Better yet is a receiver with an OTA module (I needed it last night during a bad storm).

Anyone aiming at 61.5 and 72.7 needs to be aware that they MUST use VIP or XIP receivers, all local channels on 61.5 are MPEG4, both HD and SD.

There are a very few MPEG2 channels on 61.5 like Fox Soccer plus, Arts, DOD, ALMA and 3 Angels.
