My father had a similar elusive problem, some time back and D* assigned a case number and a phone tech to over see the problem. A very professional tech came to his home and fixed it quickly. He actually diagnosed the problem (stapled cable) with out the usual trial and error parts replacement. He and I were both quite pleased.
