Is there any extra cost to activating this service, or is this part of the premiere package?

The service is free for Starz! subscribers. They haven't announced any plans to charge for it, although it now appears in the list of services on the account viewing part of the website.

No extra cost, it's part of the STARZ package. If you have TC Premier, you get it.
