Req. for difinitive list of locals coming from CES

I am looking for a difinitive list of the planned local markets coming to Dish in the near future as PRINTED on any literature from the 2004 CES. I am really looking for the complete enchilada. A scanned image if possible. I am uninterested in what some one may have said since this can be misinterpreted, misremembered, misunderstood or just plain BAD info by a rep trying to sound like he knows more than he does.
There are two or three threads dealing with this information, but each has slight variations...
One mentions Norfolk, New Orleans and Providence which from my understanding were abandoned due to uncooperative locals at least TWICE each. Another mentioned Toledo, but there is no mention of it anywhere else from any other source.
If anyone has any information please post it here and your source. I'm trying to stay ahead of the game on the EKB. I've added a few I've confirmed as planned 2004 cities.

They have the 8 more posted at CES according to the one thread.
They have a couple more verbally mentioned at CES.
I hope they can fit that all on a couple of CC charts. Unless of course, they are wasting all their CG capabilities on providing charts bashing Viacom and Time Warner/Turner.
