Well, another DISH 722k bites the dust.  I came home from work yesterday afternoon and noticed that TV 2 output had green lines.  So, I unplugged it for a few minutes and then power it up again.  At best, I will get a green LED on the front, but no audio or video out.

try TV1 on all outputs

Thank you very much for the reply.  Trust me, I have tried it on all outputs.  It is Tango Uniform!

I bet you missed Composite and ch3/4 outputs :)

Actually I have the following:

 

-Composite connected to Slingbox PRO-HD.

 

-Coaxial Channel 3 connected via RG-6 to a second TV in the same room.
