Confusion on packages

I'm looking at possibly switching up to the premier package, as I've got HBO, Cinemax, and sports channels - the question I have is, I've got the HD DVR package right now. Will I need to tack on the extra $9.99 for the HD and the extra $5.99 or $6.99 for the DVR services as well?

DVR Services are included however the HD Access fee is not.
