Sometime between 1/5 and 1/15 my wife recorded a 20 minute segment of Dr. Oz on KABC-TV.

It was a repeat of the afternoon show and had a segment she wanted to keep. A manual recording was made of the 20 minute segment overnight and it recorded fine - no reason to waste space recording the entire 60 minute program.

Now in the Recordings Playlist, it now states this will expire on 9/21.

If I click on the program and go to KEEP, the only option is to keep until it expires or until disk is full.

There should be NO FLAGS or expiration on an OTA Broadcast.

Why is this happening and is there any way to prevent it?

It's only 20 minutes. Why not just watch the program before it expires?

The OP may want to keep it forever.
I suggest that the keep until disk is full option be done.

BTW, what model DVR is this?

    Correct - keeping for reference

Never showed an expiration until recently.
