    All possibilities presented are possible.
The most common scam outside of hacking the data is simply a CSR that is pulling data of their terminal and texting it to an accomplice.

Definitely a scam as was mentioned, but this is not just new accounts. This happened to me a couple of years ago and I've been a customer for 15+ years. So there is a leak somewhere...

When this guy called, they already had access to my account and added all of the movie channels to my account beforehand to try and prove that it was legit.

They spoof the caller ID, which is extremely easy to do.

They of course tried calling me back after 8pm that night. I didn't answer, and they called over and over again. So I decided to answer and have fun with the guy. I told him that I bought the gift card and I already called DTV and got it all worked out and thanked him for giving me a great deal. The guy didn't know how to proceed after that. I wish I recorded the call.

However, it took me a few calls with the real DTV to get it all straightened out because of how much they screwed with my account.
