Pretty simple, really.

If you decide to purchase Rambo at, say, 1:00 in the afternoon, you'd be able to watch it as many times as you'd like until 1:00 the following afternoon (24 hours later).

I you wanted to watch it again, you'd have to pay $4.99 again for another 24-hour rental period.

Wow not liking this. Seems a little sketchy to me.

It applies to HD PPVs and On Demand movies only. SD isn't affected. All of this is because Hollywood studios require it. They fear that if people are allowed to have HD movies longer than 24 hours that they'll then make illegal copies of them...yada, yada, yada.

I believe it affects ALL Non-Special Event PPV... regardless of format.

I don't think it affects SD PPVs. IIRC, I remember reading something about this somewhere. I could be incorrect, but if one records an SD PPV, one will be able to find out right away.
