Can anyone tell me what channels 92, 94, 95, 97 & 98 are? They all say HD and "To be announced", but aren't even listed on the D website in the channel listings.

Maybe, but the HD PPV's in the 133 to 139 range are listed as PPVHD and have shows listed. It might be reserved for future PPV use.

They were original PPV and Special Event HD channels. MPEG2 HD. I have no idea what they're being used for now.

Old MPEG2 HD channels from 110 or 119. Being phased out anyhow so nothing to worry about.
