Your problems are not unique, unfortunately. The R15 seems to have a mind of it's own at times, and for no apparent or identifiable reason, two R15's will perform entirely differently under similar circumstances.
A couple of behavior/usage patterns seem to stand out as being associated with problems more than others. This is only from a lot of posts in this, and other, forums.
First, using searches and setting autorecords seems to be a pretty sure fired way of messing up an R15. Scheduling recordings from the guide seems to be much more reliable.
Second, setting lots of series links (approaching the limit of 50) seems to cause more problems than having fewer.
Third, making a lot of changes in the prioritizer may cause problems or hangs.
An R15 may run very smoothly for quite awhile, then start having problems. It seems once it starts having problems, the problems only grow worse over time, and the only way to recover is to do a "reset everything" or "reformat". Reset everything is a menu option that erases everything on the unit and sets it back to factory defaults. Reformat is a button pushing exercise that does essentially the same thing.
In most cases, replacing the R15 isn't going to get you anything more than doing the reset everything or reformat. In either case you lose recordings, but doing the reset everything takes a couple of minutes while getting a replacement takes several days.
We are all waiting, some more patiently than others, for DirecTV to provide a software update that really does fix the stability issues with the R15, and resolves most if not all of the recording issues, including first runs versus repeats. Keeping our fingers crossed that 10FA (for the -500) is that release, but don't know yet as it hasn't been out long enough to really tell.
