Whether you purchase from D* or a retailer, it will be leased and therefore covered under their lease agreement the same either way.
You have the AT9 already. Are there four cable runs from the dish already as well? You will need two feeds for the HR20. If you already have cable runs to feed it, then a service/install call should not be necessary. However, if you do not currently have the runs to feed it, unless you can do that all yourself, you would need an installation appointment, too.
I believe if you know you will need an installation appointment, you might as well contact D* and see if there is any deal to be had on getting a new HR20 - especially because they would throw in the installation call for free (or should!). If you are ready to bring one home, take it out of the box and just plug it in, then maybe a retailer is just as well.
Either way, the unit is covered. Also either way, you should expect that you will restart the clock on a two-year commitment upon activating the box.
