On a few occasions recently when watching a recorded program there has been a disconnect in the closed caption disconnect both DirecTV and regular.

Yesterday on one of my HR20-700s it happened about halfway through the movie Thorne. All had been well for about half the movie when the CC started running about fifteen seconds behind. I tried stopping and resuming, switching CC types...everything I could think of but nothing helped.

What might be causing the sync to fail?

In my uninformed opinion. I think it's program/broadcast specific, and nothing to do with the hardware.

My wife watches pretty much all DirecTV programming with cc turned on. When a show's captions are off, no amount of pausing, rewinding, etc, will bring it back in sync.

Most shows (aside from live shows), are pretty good.

I still don't understand the caption problems with Mad Men. One of the most watched shows in cable TV, and CC has been off/missing/skipping the last 3 or 4 years, including the most recent season that just finished. Bizarre.

I agree. The cause of the synch problem is in the source, not in the receiver. It MIGHT be the DirecTV encoder, before the signal is sent up to the satellite, but it is not the receiver in your home. Mythbusters always lags quite a bit.
