I would like to add a DVR in my bedroom but I currently don't have any coax lines in the room. I live in a 3 story house with the dish on the roof. The coax lines from the dish run down to my basement where I have a multiswitch in a dedicated box. From there, all the coax lines go out to the room locations.

The box where all the coax lines are run to/from is in the basement, on the opposite side of the house as my bedroom. My bedroom is two floors up from the basement and all 3 floors are finished. Is it possible to get two coax lines to my bedroom from that multiswitch, and if so, how the heck is that done with all the walls finished and the rooms on opposite sides of the house?

You could attempt a multistory wall fish. But you'll be hard pressed to find anyone who will attempt that. You need to open he wall in two spots on every floor.
Any way to neatly run the wire on the outside?

No, not really. When the home was built they put in 2 or 3 extra coax lines from the attic down to the box in the basement that currently aren't being used, but I don't think they are long enough, up in the attic, to reach the location in my bedroom. I believe there are 6 or 7 coax lines running from my attic (the 4 being used go out a vent in the roof to the dish) down to the multswitch, but again, I don't think there is enough slack/excess wire in the attic to get to the bedroom, and halfway down the wall. Any other options?

Can you access the attic? And can you get above the room in question. You can always extend the line with 'barrels' and then fish the lines from the attic down into the room.

Yes, I absolutely can get in the attic and the access to it is in our bedroom closet, right on the other side of the wall I want to put the TV. I was up in the attic when I upgraded to HD, feeding the two extra coax lines through a vent, to the installer. The extra coax lines are neatly looped and connected to a truss.

Do you lose signal strength when you add a barrel and extend the line with 2nd coax wire? Any idea how much an installer would charge to do this?

There is a small amount of signal loss but nothing major.
As how much the charge would be. A normal wall fish is about $65.
Or you can DIY. Get some wire, barrels and a wall plate and you should be set.

I'd install another multiswitch in the attic. Then run lines from that switch to the room in question and down to the switch in the basement.

Which switch to use depends on what dish you currently have.

Make the lines longer. Barrel connectors and more cable = problem solved.
